Participating InsurancesFor your convenience, HARTZ Physical Therapy provides direct billing to your insurance company. It is important we receive all necessary and required insurance information during your initial evaluation. It is required that you obtain a referral from your family (primary care) physician if you are participating in an HMO insurance plan. Your health insurance is a contract between you and your insurance company. Therefore, it is your responsibility to contact your carrier if there is a problem with your claim. If you have any questions about our fees, payments or insurance claim, please do not hesitate to discuss it with us.The following are insurance companies that we participate with - if you have any questions regarding insurances, please contact our office at 625-2228.
If your insurance company is not listed here, please contact us and we will try to get in network. In addition, we may provide services as an out-of-network provider for certain insurance carriers. Although this may mean lower reimbursement rates in some cases, out-of-pocket charges may not differ significantly from that of an in-network service provider. Please contact our office for more information.
